Community Care of the Lower Cape Fear provides registered nurses and social work care managers, pharmacists and other health care professionals to help you manage your health care. Our goal is to improve your health. Our services are provided at no cost to eligible patients.
Our care managers will help you learn how to take care of your chronic health problems, like asthma, diabetes, and heart failure. They will work closely with you and your doctor to understand your health care needs.
