NC County Health Rankings for 2018
The County Health Rankings & Roadmaps program ranked Community Care of the Lower Cape Fear fourth in potentially preventable 30-day re-admissions and sixth in Emergency Department Utilization out of 14 networks in the state in 2018.
What are Key Performance Indicators (KPI’s)?
• Key Performance Indicators (KPIs) show utilization and costs compared to an expected rate
• KPI expected rates are calculated based on the disease burden of the patient population
• The goal is to have KPI rates that are below the expected rates
• When our KPIs are below expected, it indicates that we are improving patient outcomes while reducing unnecessary costs
The Key Performance Indicator report takes into account the disease burden of enrolled Carolina Access Medicaid patients (excluding Medicare/Medicaid Duals and Health Choice patients). This proprietary algorithm than predicts an expected cost, number of inpatient admissions and emergency department visits for your patients based on their clinical risk. The graphs below compare these expected rates and actual rates based on paid Medicaid claims.
